Chicago beauty manufacturer cited for OSHA violations

AFAM Concept Inc., a manufacturer and distributor of beauty and personal care products, has been cited for twelve serious OSHA violations. The agency fined ​AFAM $59,700 for exposing workers to dangerous machinery, as well as fall and respiratory hazards, according to Occupational Health & Safety. In addition, AFAM was listed for one repeat and two lesser violations. 

"AFAM Concept must develop safety procedures for dangerous and known hazards," said Kathy Webb, OSHA's area director in Calumet City. She explained: "OSHA takes unsafe conditions seriously and wants to protect workers and hold employers accountable for safety."

An April 29 inspection, prompted by complaints, revealed the violations. 

In particular, OSHA claimed that AFAM did not develop procedures to protect workers from moving machine parts during service and maintenance functions, a repeat violation from June 2012. This left workers open to potential risks, including amputations, lacerations and death. The facility also did not have lockout devices on live machines, and the company failed to provide safety procedure training to its machinery workers. Furthermore, machinery with chains and sprockets were not guarded adequately to prevent workers from touching moving parts.

A list of other serious violations included lack of fall prevention equipment, railings, a respiratory protection program, personal protective equipment, as well as monitoring for employee formaldehyde exposure and training on hazardous chemicals in the workplace. Additionally, the company had no confined space entry control program to safeguard workers who entered spaces with potentially hazardous conditions.
