Falls accounted for 294 construction worker fatalities in 2013, according to OSHA.

According to OSHA's latest regional news release, Affordable Exteriors, a company specializing in roofing, windows and other outside home fixtures, has been fined over $75,000 for safety infractions.

This is the seventh time in 10 years the company has been cited, said NBC affiliate WOWT.

The OSHA investigative report details the violations at the Elkhorn, Nebraska worksite including:

  • A repeated violation for insecure elevated platforms in the work area.
  • Improper use of ladders and inadequate ladder safety training.
  • Three serious violations for exposing workers to unprotected sidings and edges of workspaces.
  • Two willful violations for worker exposure to fall hazards due to lack of fall protection.

Omaha.com said OSHA's Omaha office has a local emphasis program in place to enforce worker fall prevention. This type of outreach can include training, speeches and informational meetings, according to the U.S. Department of Labor.

Affordable Exteriors owner Rich Tiller has plans to meet with OSHA representatives to discuss the fine, said the Lincoln Journal Star. The company has 15 days to respond from the issue date.

OSHA reports stated there were 36,163 federal inspections in 2014.

The leading cause of construction worker deaths is fatal falls. Most of these falls can be prevented with proper safety training, adequate safety protections and enforced rules.

Almost 59 percent of OSHA cited construction worker deaths were caused by the "fatal four": falls, struck by objects, electrocutions and getting caught between structures.

OSHA's Omaha area director Bonita Winingham stated that the company's repeated violations are "unacceptable" and only expose employees to preventable accidents and injuries.
