Ohio plant fined $816,500 for OSHA violations

It was determined that the company had continued to expose workers to hazards, and even submitted false documentation pertaining to previous violations, claiming they had been corrected when they actually had not.  

In addition to the willful and repeat violations that put workers in serious danger, OSHA found that this company did not provide training about how to maintain machines without running the risk of amputation and other injuries.
